Rely Health is attending ACEP to engage with our clients who partners with us to ensure that their patients have support throughout the emergency department experience, including navigation support in their post-acute healthcare journey. Most Emergency Departments face challenges in scaling their case management support teams to ensure comprehensive follow-up for all discharged patients. Typically, due to scaling capacity the current model can only support the highest-acuity patients or those with long-term placement plans, such as those needing skilled nursing or rehab. Consequently, nearly 70-90% of patients leave the ED without a comprehensive follow-up plan and navigation support.
